Home-Based Palliative Care

Home care is the heart of Soul Palliative Care, offering patients the comfort of receiving medical and emotional support within their own homes. Our multidisciplinary team, including nurses, doctors, physiotherapists, social workers, volunteers, and support staff, worked together to ensure each person received timely and appropriate care.Services provided through home visits included pain relief, symptom management, nursing support, and the free provision of essential aids and medicines. Families were also supported with counseling, basic supplies, and guidance on how to care for their loved ones confidently at home. Care was extended across several communities in Chengalpattu district (Medavakkom, Madampakkom, Selaiyur), Kancheepuram district (Velachery, Porur, Narayanapuram), and Chennai (Kannaginagar). 

Regular follow-up calls and mobile-based tele-medicine helped maintain continuous support, especially for those living in remote or low-access areas. Through this approach, Soul ensured that every patient felt supported, connected, and cared for throughout their journey. 

Hospice care

Soul Palliative Care provides hospice support for patients in the final stage of life, offering a peaceful and compassionate place where they can spend their last days with dignity and comfort. Patients who can no longer be managed at home receive round-the-clock medical and nursing care focused on pain relief, symptom control, and emotional and spiritual support. The hospice provides a homely environment with nutritious food, personal care, mobility assistance, and activities such as music, reading, and gentle physiotherapy to enhance comfort. Families are encouraged to be present, and they are offered counselling and bereavement support to help them cope with their loved one’s journey.

Through this service, Soul ensures that every patient experiences compassion, peace, and respect—truly creating a “home away from home” for both patients and families.

Training and Development

Training and capacity building are central to Soul Palliative Care’s mission. To strengthen the future of palliative care, Soul conducted a series of programmes focused on creating skilled health workers, confident volunteers, and informed communities.With guidance from Pallium India, our training sessions were aligned with national standards and global best practices. Soul organised awareness classes for the public, structured training for volunteers, and skill-based sessions for nurses and other health professionals. These programmes covered essential palliative care principles, communication skills, symptom management, home-based care, and emotional support.

Through these efforts, Soul helped build a knowledgeable and compassionate workforce, while also promoting greater community understanding of palliative care.

Monthly Food Kit Distribution

Soul Palliative Care provides monthly nutritional support to extremely poor families affected by life-limiting illnesses. Many caregivers are unable to work due to the constant demands of caring for their loved ones, which makes consistent access to food a major challenge. To ease this burden, Soul distributes food kits every month to ensure that families have the essentials they need.

Each kit includes rice, dal, oil, spices, tea, sugar, oats, nutritional supplements, and adult diapers—items chosen to support both the patient’s health and the family’s daily needs. Volunteers and staff deliver these kits directly to beneficiaries, ensuring dignity, convenience, and continuity of care.Through this initiative, Soul not only addresses hunger and financial strain but also strengthens the overall wellbeing of patients and their caregivers, reminding them that they are not alone in their journey.

Rehabilitation & Assistive Devices

Soul Palliative Care offers rehabilitation services to help patients maintain mobility, reduce pain, and improve daily functioning. Through regular physiotherapy, patients receive exercises and guidance that support comfort and independence. Psychological rehabilitation is also provided to help patients and families cope emotionally and adapt to the challenges of serious illness.

To support daily living, Soul supplies essential assistive devices such as wheelchairs, walkers, sticks, and airbeds. These aids are offered free of cost to families who cannot afford them. Along with this, patients receive support for basic Activities of Daily Living, ensuring safety, ease, and dignity in their everyday life.

Blood Donation Camp

We know the value of blood as the scarcity of blood is becoming terrible nowadays. Patients of severe accidents experience the urgency of blood. Donating blood is noble work. Life can be saved by donating blood and It is also beneficial for the donor’s health. We organize blood donation camps each year. We make people aware of the importance of donating blood and convince them to make them donate their blood and make a living out of risk.

Hair Donation Camp

It is sad that cancer patients face a big problem with their hair while continuing their treatment. They lose their hair. They lose their beauty and their confidence too. We arrange hair donation camps where people can donate their hair so that wigs can be made from it for the patients who lost hair to cancer.

Home Care services

We provide home care services to patients in needs, our home care includes doctor-nurse visits, physiotherapy, supply of medicines and assistive devices, supply of food kits, educational support for children, facilitating vocational training and rehabilitation support depending on patients’ need. We have travelled to more than 30 villages in and around Chennai, and cared for more than 500 patients and their families.

 

Volunteer Training & Eligibility Examinations

We never compromise on the quality of service delivered to our patients. Hence, every volunteer at Soul is required to undergo training in Palliative care services and tested with periodical examinations so that they are appropriately equipped with the knowledge and skills to deliver top-notch services to our patients and their families
