Home-based care is the core of Soul Palliative Care, providing medical, emotional, and social support to patients in the comfort of their homes. A multidisciplinary team delivers pain and symptom management, nursing care, essential medicines, and family guidance across multiple communities. Through regular follow-ups and tele-medicine support, Soul ensures continuous, compassionate care and connection throughout each patient’s journey.

Home care is the heart of Soul Palliative Care, offering patients the comfort of receiving medical and emotional support within their own homes. Our multidisciplinary team, including nurses, doctors, physiotherapists, social workers, volunteers, and support staff, worked together to ensure each person received timely and appropriate care.Services provided through home visits included pain relief, symptom management, nursing support, and the free provision of essential aids and medicines. Families were also supported with counseling, basic supplies, and guidance on how to care for their loved ones confidently at home. Care was extended across several communities in Chengalpattu district (Medavakkom, Madampakkom, Selaiyur), Kancheepuram district (Velachery, Porur, Narayanapuram), and Chennai (Kannaginagar).

Regular follow-up calls and mobile-based tele-medicine helped maintain continuous support, especially for those living in remote or low-access areas. Through this approach, Soul ensured that every patient felt supported, connected, and cared for throughout their journey.
